What to expect for your appointment
If you are NEW to the practice, you must complete NEW PATIENT REGISTRATON FORM.
- The appointment on the day is for vaccine administration only. Please talk to your regular doctor if you have any questions or concerns prior to your vaccine appointment.
- You will be required to remain at the practice for 15 minutes following your vaccination to monitor for any side effects.
- If you cannot make it to your vaccination appointment, please contact us one day before to arrange a new appointment and so we can give your appointment to someone else. Please help us avoid any wastage. Please see our Non Attendance Policy
- Non-Medicare patients: The vaccine is free however consultaion fees apply; rebate available from your insurer or health fund.
Help us keep you, our staff, and our community safe. Please call reception before you come into the practice:
• if you are feeling unwell or have fever, cough, runny nose or other symptoms flu-like symptoms
• if you are waiting for COVID-19 test results, or have tested positive for COVID-19 or another virus
• if you are a close contact of someone with COVID-19
Booking your vaccine
Book online (using HotDoc) select the vaccine type or Call reception on (03)87865955. Non-Medicare patients must have an IHI (individual healthcare identifier) so we can upload evidence of your vaccination. You get this via MyGov. Please note that although the vaccine is free private consultaion fee will apply.

Proof of your vaccination: We will upload your vaccination details to AIR. It is very important that your details are up to date. You can get proof of your COVID-19 vaccination through Services Australia here or Digital Health here or by calling Medicare on 132 011
You can get your immunisation history statement here, You can access it via your Medicare online account through myGov or The Express Plus Medicare mobile app. If you can’t get your immunisation history statement online or need help, you can call 1800653809

New patients to the practice welcome. Please complete the online registration form prior to your appointment. Pleas bring your Medicare card and Pension or Health Care Card if applicable on the day. You may wish to email us a photo it to admin@caseygatemc.com.au
